Beginning Monday July 7, 2014, the Ohio State Patrol will be responding to most accidents within the City of Middletown. For copies of those accident reports and/or photos, please contact Ohio State Patrol at http://statepatrol.ohio.gov/crash.stm or (513) 863-4606.

As of June 4th 2018, the File a Police Report function is unavailable while we upgrade our public safety systems. At this time, accident and offense reports may not appear in online searches. To obtain information for an accident report that happened after June 4, 2018, contact Police Records at 513-425-7788. To obtain information for an offense report that happened after June 4, 2018, contact Police Records at 513-425-7794. We apologize for any inconvenience this may cause.

UPDATE: As of September 26th 2019, the City of Middletown accident reports have been disabled due to new legislation requiring telephone numbers to be redacted from accident reports. If you need to get an accident report, contact Police Records whose number can be found above.
