You may also order death certificates for deaths occurring in the city limits of Middletown. The cost of a birth certificate is $25.00 per document. The cost of a death certificate is $25.00 per document. Please select an option below.

• Birth certificates may be requested for anyone born in the State of Ohio (1908 - Present).
• Death certificates may be requested for anyone whose death occurred in the City of Middletown (1885 - Present).
• Birth certificates for newborn babies may take up to two weeks to process to ensure the newborn's data has been received by the state.
